This service assists you by increasing your capacity to manage different aspects of life including transitioning through obstacles / life-stages, mentoring or peer support, and skill development.
As life transitions or your environment changes, which can be hard to adjust to, our goal is to ensure that they remain as smooth as possible. This includes short-term and long-term support to: strengthen a care-recipients’ abilities and choices, aid in joining university or finding a job, crisis /conflict resolution and support networks. Some of Our services may include:
1. Support Coordination – Level 1 (support connection) and Level 2 (coordination of supports)
Level 1 – Support care-recipients understand their plan and connect with NDIS providers and community to increase confidence and skills to manage their plan independently
Level 2 – Assist care-recipients capacity to maintain relationships, manage tasks, and live independently by utilising a mix of supports
2. Psychosocial Recovery Coach
• Assist with recovery-orientated practice to empower and support care-recipients in making autonomous decisions, live with purpose, and stay a valued member of the community. Recovery coaches design, plan, and implement a recovery plan by working collaboratively with care-recipients, their family, and carers
3. Accommodation and Tenancy Assistance
• Assists care-recipients find and retain safe / appropriate accommodation
4. Individual Living Options (ILO)
Exploration and design – sourcing and managing the property
Support model – which is the care provided in the home
