Community Nursing Care services enable us to support your independence and collaboration in the community by delivering care that is customised, flexible, cost-effective, and specific to your health needs or preferences.

For a stronger, more-resilient, and independent you, not only does our team of health professionals & clinicians specialise at supporting varying disabilities, Boorloo Care also has trained experts in managing chronic- and metabolic- conditions.

Research indicates that the more readily available a community nurse is in a care-recipients model of support, the greater the stimulation of healing and immunity due to increased empowerment and dignity. These services are delivered directly in the comfort of your own home. Some of our services may include:

• Catheter management
• Complex bowel care
• Diabetes management
• Type-1, and type-2 diabetes
• Insulin titration
• Advanced technology – continuous subcutaneous insulin infusions (CSII) or continuous glucose monitoring systems (CGMS)
• Peripheral neuropathy
• Limb amputations
• Dialysis
• Cognitive limitations
• Cancer
• Enteral feeding
• Epilepsy and high-risk seizure management
• General nursing care (hygiene, showering, dressing)
• Severe dysphagia
• Subcutaneous injections
• Tracheostomy management
• Ventilator management
• Wound and pressure injury management
• Medication management

It is important to understand that the NDIS will only fund Community Nursing Care services that are related to your disability, and will not cover any expenses related to medication, general medical treatment, hospitalisation, or surgery.

The Community Nursing Care provided by Boorloo Care involves advanced care or training supplied by Enrolled Nurses, Registered Nurses, and Clinical Nurses to NDIS care-recipients both at a standard or general, and high-intensity level of support. Our restorative and prophylactic Community Nursing Care services help restore your health, whilst providing your family carers with respite and support. We will prevent your re-admission into hospital and other acute-care settings, so that you can continue to live life as autonomously as possible.
